HOW DO I....
Obtain A Repeat Prescription?

You will be given a repeat prescription if your doctor thinks it is advisable. At regular intervals your doctor will need to see you before repeating your prescription so that your treatment can be monitored. Repeat prescriptions will have a detachable sheet listing the medicines that have been agreed by your doctor. Requests can be made by ticking the items that you require and either posting them to the surgery (please include a stamped addressed envelope if you wish us to post your prescription to you), or placing them in the box provided by our reception. Prescriptions will usually be ready for collection two working days later. Our legal obligations do not allow us to accept repeat prescriptions over the telephone as this can lead to dangerous errors.

THIS FORM IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
